Introduction: Achieving long-term closure of nasal septal perforations remains one of the most challenging goals in nasal surgery. Diffrent types of techniques were described in the litrature with variable results. The aim of this study is to describe our surgical technique for repairing septal defect. Methods: We present eleven patients who underwent an endoscopic repair of anterior septal perforations with unilateral septal pedicled flap by anterior ethmoidal artery. Post operative symptoms were measured. The patients were followed for a period of 12-132 months (median, 51 months). Results: All cases of septal perforation were closed with this technique withoud intraoperative incidents and remained closed during the follow-up duration. There were no complications encountered after surgery. Conclusions: Closing a perforated nasal septum through an endonasal technique can be achieved with a unilateral mucosal flap based on the anterior ethmoidal artery.
