Solitary fibrous tumors (SFTs) are rare mesenchymal neoplasms with variable malignancy potential. We report a 69-year-old man with elevated PSA and an asymptomatic pelvic solid mass (42 x 32 x 48 mm), found on imaging between the bladder and anterior prostate. CT-guided biopsy confirmed an intermediate/low-risk SFT per WHO 2020. The lesion was successfully removed via robotic prostate-sparing surgery. Accurate histopathological diagnosis is key to avoid overtreatment, as SFTs can mimic more aggressive tumors. Complete excision offers good prognosis, though long-term follow-up is warranted due to potential for recurrence or progression.
